The Patient Portal offers established patients an easy and private way to access their medical information online, so you can view your personal health information whenever and wherever you have access to the Internet!

With the Patient Portal, you’ll have access to:

• Medication: request prescription refills
• Medical Records: view your personal health record
• Education: receive educational material
• Messages: send messages to clinical staff
• Reminders: receive health maintenance and appointment reminders

How do I access to the Patient Portal?

If you already have a username and password, click here to access the login screen.

If you do not have a username and password, or if you have any other problems, please call our office at (708) 799-5520 and we will create an account for you or provide assistance.
